Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
Air Bag Inflator Rupture
The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers.All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers.Nonetheless, at ODI's insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation.The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures.As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.Takata's initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida.Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas.Takata's testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions.During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina.Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls.Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity.Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing.All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas.The investigation now includes all manufacturers and vehicles known to be affected at this time.ODI's investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.The five ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10537899, 10568848, 10585224, 10605877, 10651492

DURING TWO OF 13 NHTSA RESEARCH & DEVELOPMENT COMPATIBILITY CRASH TESTS, THE DRIVER-SIDE FRONTAL AIRBAG FAILED TO PROPERLY DEPLOY DUE TO A LARGE TEAR IN THE AIRBAG FABRIC.BOTH R&D TEST VEHICLES THAT CONTAINED A FAILED AIR BAG WERE 4-DOOR, 4-CYLINDER (L4) VEHICLES.SUBSEQUENT STATIC AND DYNAMIC TESTS CONDUCTED BY HONDA REVEALED THAT DURING SEVERE CRASH CONDITIONS, THE AIRBAG FABRIC CAN SNAG THE INSIDE SURFACE OF THE AIRBAG MODULE COVER AT THE EMBLEM ATTACHMENT POINTS.HONDA REPORTS THAT THE AIR BAG MODULE AND COVER DESIGN ARE UNIQUE TO THE 4-DOOR, 4-CYLINDER SUBJECT VEHICLES.ON NOVEMBER 17, 2004, HONDA NOTIFIED THE AGENCY THAT IT WOULD RECALL 228,932 MODEL YEAR (MY) 2004 AND 28,428 EARLY-MY 2005 ACCORD L4 SEDANS (UP THROUGH EARLY-OCTOBER, 2004).AS PART OF THE RECALL REMEDY, HONDA WILL INSTALL AN ADDITIONAL PIECE OF PROTECTIVE FABRIC/FLAP BETWEEN THE AIRBAG AND THE INSIDE AIRBAG MODULE COVER.SEE NHTSA RECALL 04V551 FOR DETAILS.IN ADDITION, THERE ARE APPROXIMATELY 250 EXPORTED ACCORD VEHICLES INCLUDED IN THIS RECALL. THE V-6 MODELS AND THE 2-DOOR, 4-CYLINDER MODELS ARE NOT INCLUDED BASED ON DIFFERENT AIR BAG AND AIR BAG MODULE COVER DESIGNS.THIS INVESTIGATION IS CLOSED.

Odometer Fraud. The contact purchased a 2004 Honda Accord. It was discovered that there was a mileage discrepancy after the purchase. The vehicle was a dealer sale. At the time of the purchase, the mileage was approximately 134,000. The contact stated that while attempting to resell the vehicle, the contact was informed by an individual interested in purchasing the vehicle that the mileage reported might not be the actual mileage. The contact was advised to check the Carfax Report. While checking the Carfax Report, the contact noticed two potential reports of odometer fraud with the vehicle. The Carfax Report indicated that in 2023, the mileage was reported to be approximately 251,000. The contact had not filed a report with any other agencies.

I have 22 recalls in my Jerry app and also on here. Have problems when first starting up and shifting just got from a dealer in Ohio. Maybe a year and a half ago my name is Tracy Harney my address is 702 Inverness place ft thomas Ky 41075 also my air bag light is on and my headlights are not very visible at nite wondering if all recalls on vehicle were fixed. The transmission in my 2004 Honda Accord V6 3.0L is experiencing a serious safety defect. The transmission overheats while driving, begins slipping, and eventually the vehicle will not move at all. When this happens, I lose the ability to accelerate or maintain speed, creating a dangerous situation for me and other drivers. I have to pull over and wait for the transmission to cool before the car will move again. This failure matches the symptoms described in Honda’s prior transmission recall 04V176000 (overheating and gear failure due to lubrication issues). I have contacted Honda about this issue, and they opened Case #16008926, but the problem is ongoing and has not been resolved. The problem is reproducible and happens repeatedly after the transmission warms up. Warning symptoms include overheating and severe slipping before total loss of movement. This is a major safety hazard because the vehicle can suddenly lose power in traffic.

I bought a used 04 v6 accord with 84,000 on it I bought it for $7500 that was in May/June 2021 he was going on and on about the air conditioner and the jbl radiothe About the time it started getting cold I noticed problems with it not wanting to go when put in drive unless it was warmed up enough didn't really think much about. in the colder months the next year it would start doing some jerking problem and then it started messing up like when you are slowing down for a red light but then it turns green and you give it the gas but it just wines like it was in neutral but it was still in drive so I would have to pull over to the side of the road coasting but once I got to a lower gear like first gear I could hear it and feel it go into gear and then when I gave it the gas it would go. Google found a 4.3 million recall on that 04 v6 tranny... mad people same problems they had transmissions replaced somewhere saying to change the transmission fluid every three months just with 3 quarts until the torque converter was flushed out and so on well I had just bought this vehicle pretty much put my savings into it. Apparently the recall was for shavings from the transmission getting in the linkage causing it to pop out of gear if not that bad it could be fixed but if it was bad transmission need replacing no recall on my car if it has same safety problem why no recall. my childhood friend told me it was a transmission it got so bad I had to park sitting 2yrs i have no one to work on it I don't have the funds to pay a garage my friend had a heart attack passed away some say shift solenoid, I know that seller knew there was a problem he spent $600 yea no revall for him he ripped me off but at such low miles there's got to be a reason for this barely used car to be such a safety hazard...my dad told me to write you guys what can it hurt he said the car has been sitting because I have no money to pay to get it fixed I live on SS. The contact owns a 2004 Honda Accord. The contact stated that while attempting to make a right turn, the air bags deployed independently. No warning lights illuminated. A dealer or independent mechanic was not contacted. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was not repaired. The contact's fiancé sustained injuries to the face, left eye, causing her to have a black eye, and her chest and the contact sustained injures to left side of his face and his left hand. The manufacturer was not notified of the failure. The contact was relating the failure to NHTSA Campaign Number: 18V268000 (AIR BAGS); however, the VIN was not included in the recall. The approximate failure mileage was 147,486.

Odometer Fraud. The contact purchased a 2004 Honda Accord. The contact stated that upon retrieving the vehicle Carfax history report, it was discovered that there was a mileage discrepancy. The vehicle was a private sale. At the time of purchase, the vehicle mileage was 156,100; however, the Carfax history report indicated that the mileage was 278,000.

2004 Honda Accord V6 178000k miles Car was just driving fine through all gears then out of nowhere as I was driving on the freeway going approximately 65 mph the car violently jerked because the transmission locked up and my car broke like if i pulled the emergency brake almost causing me to crash into another vehicle. The (RPM) then went up to redline. I pulled over restarted the car and as I tried to drive off, the cars (RPM) was way too high for example if I drove 40mph the (RPM) was at 4,000 revolutions witch can cause damage to the engine, If i went 60 mph and the (RPM) was at redline. So I had a friend tow the vehicle. Once home I drove around the neighborhood and the car won't switch gears it stays at 1st gear ( hence the high RPM) and it violently jerks the whole car very badly I'm assuming when it's supposed to switch gears. I got the following codes ( P0741, P0747) I checked all the solenoids with a professional scanner I was able to activate all the solenoids and all passed the test. Every single one clicked and showed they where in good condition. The next day I go drive it and it drives normal through all gears for an hour and then again it starts jerking violently / braking. I then replaced the solenoids and still the same problem. I did some research online and it seems a lot of people have trouble with these transmissions for this year I'm surprised it isn't recalled as it is very dangerous.

SUSPENSION
This SKU is a Front Upper Ball Joint. The customer communication requested return of unsold inventory to inspect for the outer diameter of the ball joint housing being too small. The outer diameter being too small would prevent the part from being installed, and would therefore have no impact on the end user besides time to complete installation.
